About This Assessment

Following a serious motor vehicle accident, many injured Ontarians require ongoing assistance with activities of daily living β€” bathing, dressing, meal preparation, mobility, medication management, and supervision.

The Attendant Care Assessment, completed using the prescribed Form 1, quantifies this need and establishes the monthly attendant care benefit payable by the auto insurer.

Our Form 1 assessments are completed by Occupational Therapists and Registered Nurses specifically trained in SABS regulations and the rigorous methodology required for defensible reports.

The Process

Step 1 β€” Pre-assessment Intake: Our intake coordinator collects your referral, records, and insurance documentation. We confirm coverage before scheduling.

Step 2 β€” The Assessment: You meet with our specialist for a thorough examination, history review, and validated testing. Length varies based on the complexity of your case.

Step 3 β€” Comprehensive Report: Within 2 to 4 weeks, you and the requesting party receive a detailed, defensible report with diagnostic findings, causation analysis, and clear recommendations.

Funding & Insurance
