In providing healthcare services, we collect personal health information directly from you and, with your consent, from other sources such as referring physicians, insurance companies, and previous healthcare providers. This includes:
- Identification details β name, address, date of birth, OHIP number, contact information
- Health history β past medical conditions, surgeries, medications, allergies, family history
- Clinical information β assessment findings, treatment notes, progress reports, test results
- Insurance details β accident benefits, WSIB, extended health plan information
- Communications β emails, phone messages, and correspondence with our team
2. How We Use Your Information
Your personal health information is used solely for purposes related to your care and the operation of our clinic, including:
- Providing assessment, diagnosis, and treatment
- Communicating with other members of your care team
- Processing insurance claims and managing billing
- Reporting to insurers, WSIB, and legal counsel as authorized by you
- Quality improvement, education, and clinical audit (de-identified where possible)
- Meeting legal and regulatory obligations under PHIPA and professional College standards
3. Who We Share Information With
We disclose your personal health information only with your express or implied consent, or as required by law. Common disclosures include:
- Your healthcare team β physicians, specialists, and rehabilitation providers involved in your care
- Insurance companies β auto insurers, WSIB, extended health, and disability insurers funding your treatment
- Legal counsel β your lawyer if you provide written authorization
- Regulatory bodies β professional Colleges in cases of investigation or audit
- Courts and tribunals β when required by subpoena, court order, or LAT proceedings
We do not sell, rent, or trade your personal health information under any circumstances.
4. How We Protect Your Information
We use reasonable administrative, physical, and technical safeguards to protect your information, including:
- Encrypted electronic health records with role-based access controls
- Secure physical storage of paper records in locked premises
- Staff confidentiality training and signed agreements
- Secure data transmission for insurance billing and external communications
- Regular audits, breach response procedures, and policy reviews
5. Your Rights
Under PHIPA, you have the right to:
- Access your personal health information held by our clinic
- Request corrections to inaccurate information
- Withdraw or modify your consent to specific uses and disclosures
- File a complaint with our Privacy Officer or the Information and Privacy Commissioner of Ontario
